AI receptionist for physical therapy clinics that books evaluations

Written and reviewed by Rafe Mirza6 min read

Picture an outpatient clinic at lunchtime on a Monday. Two therapists are on the gym floor, one patient is on the bike, another is working through a band routine, and the person at reception is on hold with an insurer about an authorization.

Meanwhile a woman three weeks out from knee surgery is calling with a referral in her hand. Her surgeon told her to start therapy this week.

She will try two clinics. She will book with the one that answers.

Example
  1. The patient calls or messages
  2. It works out the visit type
  3. It gathers your intake basics
  4. The evaluation is booked
  5. Staff take over the paperwork

What is an AI receptionist for a physical therapy clinic?

Short answer: it is software that takes your clinic’s calls, texts and website messages when the team is tied up, holds a normal conversation and books the person into your schedule. It is upfront that it is an AI, follows the answers you approve, and sends insurance, authorization and clinical questions to your staff.

In physical therapy, most new calls are one of a few things: a physician referral, someone booking an evaluation on their own, or an existing patient moving a session. Each needs a different next step, and the AI follows the path your clinic sets for each.

One thing it is not: an AI scribe. It does not write or touch clinical notes. Plenty of software sold as AI for physical therapy does that job; this is about getting patients through the door and keeping them booked.

Think of the dispatcher at a taxi firm. They do not drive the car. They take the call, work out where you are going, and put you with the right driver.

Why do physical therapy clinics lose referrals on the phone?

Reception in a therapy clinic is rarely just reception. The same person checks patients in, chases insurance verification, handles intake paperwork and answers the phone. When an authorization call takes twenty minutes on hold, everything else waits.

Referred patients are often the ones who slip. A physician referral tells someone to start therapy. It rarely tells them where. If your clinic does not answer, the next clinic on the list does, and the referring office may never know the patient went elsewhere.

Then there is drop-off. Patients who cannot reach you to move a session tend to skip it, and a plan of care with two missed visits in a row is at real risk of ending early.

Hold music has never once improved anyone’s range of motion.

Example

  • Unanswered calls during clinic hours last month: ___
  • How many were new patients or physician referral calls: ___
  • Average value of a completed plan of care at your clinic: ___
  • Sessions missed after a patient could not reach you to reschedule: ___

How does it work during a clinic day?

Here is one referred patient from the first call to their evaluation, once your clinic has agreed the rules with us.

  1. The patient calls or messages

They ring during a packed afternoon or text after work. The AI replies, explains that it is the clinic’s AI assistant and finds out what they are after.

  2. It works out the visit type

Physician referral, self-referred evaluation, existing patient reschedule or a general question. Each follows the path your clinic set.

  3. It gathers your intake basics

Name, contact details, the referring provider, the body area in the patient’s words and their insurance carrier, so your team can verify coverage. It does not ask about medical history beyond what you approve.

  4. The evaluation is booked

The patient picks from open evaluation slots with the right therapist, and the evaluation is written into your schedule, intake notes included.

  5. Staff take over the paperwork

Front office staff pick up from there: insurance verification, any authorization and the forms. Questions the AI left open come with a summary.

We set it up, update the answers whenever hours or therapists change, and look over its work with you on a set rhythm, so your team is never left maintaining it.

How does it handle physician referrals and plan of care visits?

A physician referral deserves speed. The patient has been told to start therapy and is motivated now. The AI replies to a referred patient while that motivation is fresh, offers evaluation times and notes the referring provider so your team can send the usual updates.

Plan of care visits are a different rhythm: two or three sessions a week for several weeks, and life gets in the way of some of them.

When a patient needs to move a session, they can text the clinic, see open times with their therapist and rebook. If they say they want to stop because they feel better, or because it is not helping, the AI does not try to talk them round. It passes the message to their therapist, who knows whether stopping early is a problem and can call them.

Your no-show policy can be part of the conversation too. The AI can remind a patient of it, in your wording, when they cancel late. It does not apply fees or make exceptions; your staff do.

What about insurance questions, after hours and urgent concerns?

  • Insurance, authorization, visit limits and copay questions belong to your staff every time. The AI collects the carrier name at intake so they can verify, and never quotes coverage or costs.
  • Whether a patient needs a physician referral depends on their plan and where they live, so the AI passes that question to staff instead of answering it.
  • After closing, the AI answers 24/7. A new patient can book an evaluation at night, and your team sees the intake details in the morning.
  • A post-surgical patient describing new swelling, fever or a fall, or any other warning sign you list, reaches staff right away or hears your clinic’s own instructions, unchanged.
  • Anyone who would rather talk to a person is connected to your team during opening hours, along with what they have said so far.

The honest catch: insurance verification and authorizations will not get faster because of this. Those stay with your team, and the best it can do is hand them complete intake details so the calls go faster.

Where do bookings land in your clinic’s systems?

Appointments are written into your physical therapy practice software, so evaluations and plan of care visits share the schedule therapists glance at between patients. The intake details ride along with each booking.

Many therapy systems combine scheduling, documentation and billing. The AI works with scheduling and contact details only. Tell us which system you run during the demo and we will say plainly what it can link to.

  • Evaluations it booked, with referral source and intake details
  • Sessions it moved, and late cancellations noted against your no-show policy
  • Insurance and authorization questions passed to staff, each with a summary
  • Messages about stopping care passed to the treating therapist
  • Regular results reviews with someone from AgentBlue

Frequently asked questions

Can an AI receptionist book physical therapy evaluations?

Yes, into the evaluation slots you choose, with the intake details your team needs attached to the appointment.

Does it write or read clinical notes?

No. It handles calls, messages and scheduling only. Documentation stays in your own system with your therapists.

Will it answer questions about insurance coverage?

No. It takes the carrier details so your staff can verify coverage, and passes every coverage, authorization or cost question to them.

How does it handle patients sent by a physician?

It books the evaluation, records the referring provider and passes the details to your team, who handle any paperwork the referral needs.

Can patients reschedule plan of care sessions by text?

Yes. They text the clinic, see open times with their therapist and move the session, and the change shows in your schedule.

Will patients know it is an AI?

Yes. Every conversation opens by saying so, and a patient who wants a person is connected to your team.

How long until a physical therapy clinic is live?

Live in about 2 weeks after we receive access to your systems. We handle the build and the scheduling connection, and your team approves the answers.
